He was predeceased by: his parents, Henry G. Lewis and Erma May Lewis (Ridge); and his brother Greg Lewis of Flowermound, Texas.. He is survived by: his former wife Cheryl Ann Hampton; and his brother Denny Lewis of Augustus.
