He is survived by: his girlfriend Kaitlyn Beitel of Troy; his parents, Michael Jazkubczak and Brenda Jazkubczak (Lewis) of Troy; his sister Katherine (Caleb Jones); his nieces and nephews, Jedidiah and Elijah; his grandparents, Bill, Laura Murphy of Eolia and Donna Lewis of Troy; and his aunts and uncles, Dana Barker (Ron) of Eolia, Jim (Laurie Messina), Elizabeth (Jeff Danskin), David Murphy (Meredith) of Eugene and Rhonda Lewis (Kathy Lewis) of Old Monroe. He is also survived by cousins, other relatives and friends.
