He is survived by: his daughters, Amy Willbanks (Craig) and Ashley Lewis (Travis); his grandchildren, Corbin, Kayden, Easton, Brayson, Preston, Tallen, Owen and Cecelia; his former spouses, Jeanna, Tony and Jim Mazzio; and his son Phil Mazzio.
