He was predeceased by: his parents, Albert Forman and Mable York Evans. He is survived by: his wife Carolyn Crockett; his daughters, Laura Shields (Tim Stout) and Anna Forman; and his granddaughter Savannah Shields Binder (Nick).
Memorials to the Green Hills Animal Shelter and they may be left with the mortuary.
