He was predeceased by: his parents, Phyllis Hazelwood-Siler and George W. Hazelwood; and his nephew Curtis W. Hazelwood. He is survived by: his siblings, Linda Hazelwood-Wheeler and Larry W. Hazelwood (Judith Y. Hazelwood); his niece Deanna Haushahn; and his nephews, Ryan Simmons and David "Woody" Hazelwood.
