He was predeceased by: his parents, Nicholas Polyak and Cecilia Polyak; and his sister Mary Alice Polyak. He is survived by: his wife Valerie Polyak of Sunrise Beach; his son Nick Jr. (Lori); his grandchildren, Emily and Ellie; his daughter Beth Ann Nichols (David) of Wentzville, MO; his granddaughters, Claire Roebuck (Dylan) of Columbia, MO and Gail Nichols of Springfield; and his son Steve of Sunrise Beach. He is also survived by other relatives and many friends.
