Johnson (Heather) of Pennsylvania and Andrew Johnson of Sullivan, MO; his parents, Jay L. Collins and Norma J. Collins (Bullock) of Sullivan, MO; his uncles, Dwight Collins (Joy) of Jonesburg, MO, Norman Bullock (Donna) of Salem, MO and Roy Bullock (Joyce) of Dayton, TX; and his aunt Ramona Harris of Maryland Heights, MO. He is also survived by numerous other cousins and relatives.
