He was predeceased by: his parents, Michael Cholak and Anna Cholak (Toth); and his siblings, JoAnn Cholak, Patricia Weaver, Phillip Cholak and Michael Cholak, Jr..
He is survived by: his son Anthony Cholak (Carrie); his grandchildren, Blake Cholak, Canon Cholak and Chloe McCoy; and his sisters, Veronica Powell and Geniveve Pinter (Robert). He is also survived by loving nieces, nephews, and cousins.
