He was predeceased by: his parents, Russell Cook and Jennie Cook (Keene); his wife Phillis June McCrary; and his son Scott. He is survived by: his sons, Thomas Cook (Wendy) of Eugene and David Cook Springfield (Lauren); his grandchildren, Evan, Sarah, Tuesdae and Heather; and his sister Jane Hill (Richard) of Marion, IL. He is also survived by many nieces and nephews.
