He is survived by: his parents, William Michael Chastain of Shell Knob, MO and Emily Marie Jurzynski of Beacon Falls, CT; his sister Jolene Brooke Chastain of Shell Knob; his great-grandparent Linda Capullo of Norwich, CT; his uncle Derek Nelson of Golden, MO; his aunt Tiffany; his cousin Emma Obrien of Shell Knob; his aunt Alina; his cousin Robbie Gray of Exeter, RI; his great-grandparent Raymond Jurzynski; his grandparents, Jeffrey Jurzynski (Sharon) and Helen Kelly (Joel) of Beacon Falls, CT; his aunt Jenny Rowland (Eddy) of Seymour, CT; and his uncle Jeffrey Jurzynski of Oxford, CT. He is also survived by aunts, uncles, cousins and friends.
In lieu of flowers, memorial contributions can be made payable to Fidelity Investments for his loving sister Jolene's college fund and sent directly to White Funeral Home P.O. Box 890, Cassville, Missouri.
