He was predeceased by: his parents, Robert E. Lee and Anna Marie Ward Forsythe. He is survived by: his wife Bettie K. Forsythe of Scott City, Missouri; his children, Natalie D. Mays (Tony) of Cape Girardeau and David W. Forsythe (Julie) of St. Charles, Missouri; his sister Sharon K. Marchbank of Kansas City, Missouri; and his grandchildren, Kelsey N. Mays, Chloe N. Mays, Emily M. Forsythe and Ethan M. Forsythe.
