He is survived by: his siblings, Terry Middendorf of Murphysboro, Mike Boly (Annette) of New Albany and Carolyn Harmon (Joe) of Herrin, Illinois.; his nieces and nephews, Brent Middendorf (Jess), Staci Middendorf, Luke Boly, Franny Boly, T. J. Boly, Emma Cooper (Calvin), Thomas Boly, Jeanne Schneider (Robbie) and Jodi Harmon; his great-nephew Devin Eovaldi; and his great-nieces, Maebrie Middendorf and Sadie.
