He was predeceased by: his grandparents, Erma Campbell, Jessie Leon Campbell and Opal M. Polsgrove. He is survived by: his parents, Gregory Polsgrove (Diana Queen) and Linda Polsgrove (Campbell); his daughters, Caressa Barron and Cheyenne Smith (Justin); his grandchildren, Gunner and Brooklyn Smith; his brother Gregory Charles Polsgrove; his stepbrother Jon Roberts; his grandfather Charles Polsgrove; his great-grandparent Betty Hankins; his niece Miranda Polsgrove; and his nephews, Tyler Sisk and Corbin Polsgrove.

In lieu of flowers, the family suggests memorial contributions to the charity of the donor's choice.
