He was predeceased by: his sister Kayla Sue Johnson; and his grandparents, Hosea Johnson, Earl Roger and Ethel Roger. He is survived by: his parents, Craig and Debra; his son Jaxx; his grandmother Pearlie Johnson of Eunice; and his parents-in-law, Morrell Myers, Carolyn Myers of Thomasville, Rhonda Dietz of Montgomery, James Johnson of Eunice, Philip Dietz, Tyler Elliott and Patrick Dudzinski.

In lieu of flowers, expressions of sympathy may be made to the Jaxx Johnson Educational Fund.
