He is survived by: his parents, Pavel Formuzan and Praskovia Formuzan; his spouse Viktoriya Sarioglo; his sons, David Matthew and Mark Jacob Formuzan; and his siblings, Vyacheslav Formuzan (Mila), Daniel Formuzan, Lilia Chernioglo (Vitaliy) and Marina Sarkisyan (Samvel).
