He was predeceased by: his son Matthew Aaron Collins; his parents, Ellsworth Collins and Jeanetta Collins; his nephew Jeremy Collins; and his parents-in-law, Stanley and Bonnie Jeppesen.
He is survived by: his spouses, Sheryl Jeppesen and Sheryl J. Collins; his daughters, Rebekah Jean Collins and Lesley Delores Collins; and his siblings, Anita McGhee, Sandra Avery, Chester L. Collins, Robert Ray Collins, Rita Kay Wheeler (Jon), Etta Jean Lewis (Leecinda), Anthony Collins, Sr. (Janet Baker) and Wanda Smith (John). He is also survived by numerous nieces, nephews, and extended family.
