He was predeceased by: his grandfather Herbert L. Hubbard. He is survived by: his parents, Michael Horn and Michelle Van Horn (Hubbard); his sister Natalie Van Horn; and his grandparents, Joe, Lonna Van Horn (Gooden) and Lou Hubbard (Gary). He is also survived by aunts, uncles, great-aunts and uncles, cousins, and many beloved friends.
