He was predeceased by: his parents, Charles Ham and Lota Ham; his siblings, Kirby and Sandy; his nephews, Eric Christgen and Michael Ham; and his niece Cheryl Morris.
He is survived by: his family, Jason, Doug and Dustin; his wife Dixie Lee Pryor; his sons, Jason (Cydney), Douglas (Kristin) and Dustin (Miranda); his grandchildren, Nolan, Grace, Josie, Ella, Lydia, Grady and Leo; his brother Charles "Charlie" Dwayne Ham (Linda); his sister-in-law Pattie Ham; his aunt Tina Hackett; and his uncle Larry Ham (Barbara). He is also survived by several nieces, nephews, extended family and friends.
In lieu of flowers, suggested memorial contributions may be made to the American Cancer Society, Children's Mercy Hospital, or contributions may be made in Jerry's honor to a charity of the donor's choice.
