He was predeceased by: his mother Mildred Warner; his daughter Tiffany Michelle Cook; and his brother Jackie Warner. He is survived by: his wife Donna Cook; his daughter Renee Lynn Smith of Piedmont; his sister Shirley Chalker of Florissant; and his grandchildren, Tiffany, Joey, Elijah and Presley.
