He is survived by: his parents, Johnny Cook and Myrtle Fulbright Cook; his wife Helen; his children, Keith Cook (Janice) of Patton, MO, Daniel Cook (Rhonda) of Cape Girardeau, MO and Diane Cook of Jackson, MO; his grandchildren, Ashley Mayfield (Jeremy), Bryan Cook, Nicholas Coppock, Dustin Cook and Maile Cook; his great grandchildren, Hunter Mayfield, Miley Mayfield and Jase Mayfield; his mother-in-law Clarence Allen; his siblings, Charles, Clyde, Eula Hoehn (Sis), Verla Cook, Wilma Dailey and Louise Kowniak; and also Noma Allen. He is also survived by several brothers-in-law, sisters-in-law, nieces, nephews, cousins and friends.
In Lieu of Flowers donations are requested to the Saint Francis Hospice.
