He was predeceased by: his father Thomas Levy (Joe); his stepfather Gary; and his niece Anastasia. He is survived by: his mother Nancy Levy; his siblings, Robert (Janel), Jeff (Valerie) and Wendy; his aunt Virginia; and his nieces and nephews, Camille, Tristan, Karina, Allie, Mallory and Everett.
In lieu of flowers, donations may be made in Mike's memory to Toys for Tots. Toy donations will be collected at Mike's celebration on October 15 as well.
