He is survived by: his wife Mary Ann; his brother Bill Carmichael (Shirley) of Brighton, Colorado; his daughters, Shanna Davis (Bill) of Carrollton, Illinois and Melissa Massey (Brent) of Bella Vista, Arkansas; his grandchildren, William Davis, Emily Kirkpatrick (Trevor), Ashlyn Massey and Emma Massey; and his nieces, Kristen Ostema (Glen) and Kelly Carmichael.
Memorial contributions in Ortis' memory are suggested to First Baptist Church Ozark Building Fund.
