He was predeceased by: his parents, Floyd Lee and Genevieve; his wife Carol Sue Burgess; his son James Michael "Mike"; and his wife Dolores Anne Caddell.
He is survived by: his children, Karen Suzanne, Heather Anne and Keri Lynn; his grandchildren, Anna, Jay, Luke Standaert, Madison and Will Myers; his sister Patti Floyd (Berl) of Nixa; his nieces and nephews, Tara Mulholland, Stephanie Hassell (John), Chuck Burgess (Rene) and Jeff Floyd (Missy); and his friend Sheila North. He is also survived by friends and other relatives in addition to his children and grandchildren.
