He was predeceased by: his parents, James Madison and Mary Hertz Cook; his wife Nancy Leathers; his sisters, Peggy Sweezer and Nancy Stuck; and his granddaughter Lily Covington.
He is survived by: his children, Paul Cook (Betty) of Prairie Home, Stephanie Cook of Boonville, Darrell Heath Cook of New Franklin and James Cook (Lori) of Houston, TX; his sister Marcia Nichols; and his grandchildren, Sophie Cook, Nathan Covington, Brigette Hampton, Becca Cook, Ryan Nowlin, Breanna Baker, Alyssa Murry, Brysen Thompson, Allie Cook, Amanda Cook and Mackenzie Cook. He is also survived by several great-grandchildren.
