He is survived by: his mother Katherine Smith (Goodman) of Cabool, MO; his wife Brenda Blake; his daughters, Alyssa Jean of Vienna, MO, Ashley Danielle (Michael) of Houston, MO, Allison Elizabeth of Springfield, MO and Alaina Taylor of Springfield, MO; his grandchildren, Joseph, Miranda and Dawson Collins of Houston, MO; his siblings, Vickie Smith (Ed) of College Station, TX, Kelly Cochran (Scott) of Springfield, MO, Natasha Smith of Branson, MO. and David Hartlage of New Salisbury, IN; his father-in-law Gene Blake of Summersville, MO; and his siblings-in-law, Kandra Kirker (Jerry) of Summersville, MO, Kimberly Courtney (Bruce) of Rolla, MO and Darrin Blake (Robin) of Springfield, MO.. He is also survived by nieces and nephews who thought their Uncle.
