He is survived by: his wife Tina; his children, Jeremy Phelps of Kansas City, Dakota Phelps of Chillicothe, Joshua Phelps of Chillicothe, Dallas Hines of Mooresville, Amber Williams of Richmond and Christa Phelps of Chillicothe; and his sister April Butler (James) of Braymer.
