He was predeceased by: his parents, Harold Wormington and R. G. Wormington (Hawkins). He is survived by: his significant other Sheree Halliburton of Monett; and his family, Joe Hawkins of Monett, Brad Hawkins of Monett, Terry Hawkins of Purdy and Rachel Aldridge of Cassville.
