He is survived by: his father Gary Wellbaum; his wife Nikole West; his sons, Tyler Culbertson, Jeremy Worm (Crissy) and Michael Wellbaum (Kristen); his grandsons, Wyatt and Waylon; his brothers, Shelby Wellbaum, Dustin West (Christy), Matt Bailey and Jake Bailey; his aunt Debbie Stewart (Tom); his niece Brelynn Wellbaum; his nephews, Gavin Bailey, Drake West, Josh West and Liam West; his cousins, Jamie McKinley (Brad), Dylan Stewart (Breann) and Dustin Stewart (Jenna); and his mother-in-law Loretta Bailey.
In lieu of flowers, memorial contributions are suggested to Shriners Hospitals for Children or the Masonic Lodge in care of the funeral home.
