He was predeceased by: his parents, James L. Hazlett, Sr. and Joanne Walcott Hazlett; and his sister Janice Wright. He is survived by: his wife Shelley Hartman; his children, Mikey Torres, Dan Torres, Misti Hartman, Jennifer Hazlett and Ashton Hazlett; his siblings, James Hazlett, Jr., Jay Hazlett, Zach Hazlett, Jeana Hazlett and Lizzie Phillips; his parents-in-law, Wayne Hartman and Glenda Gerry; and his stepmother Carolyn Hazlett. He is also survived by grandchildren and nieces and nephews.
