He is survived by: his girlfriend JoAnn Peters of Memphis, Missouri; his children, Andrew Middleton (Ranée) of Memphis, Missouri and Shawnna Forck (Frank) of Taos, Missouri; his grandchildren, Andrea Middleton of Warrensburg, Austin Middleton of Peru, Keregan Middleton of Memphis, Laramie Jackson (Jeff) of Lancaster, Cheyenne Garnett (Jacob) of Fulton, Derek Forck of Taos, Dillion Forck of Denver and Dalton Forck of Taos, Missouri; and his brother Dick Middleton (Cathy) of Downing, Missouri. He is also survived by 5 great grandchildren, many nieces, nephews, cousins and many, many good friends.
