He was predeceased by: his mother Carole Jean Cook Welborn. He is survived by: his father Michael Bruce Welborn; his spouse Kim Eapmon; his daughters, Cheyenne Palmer and KaeLeigh Mallady; his grandchildren, Easton and Hannah; and his mother-in-law Frieda Eapmon. He is also survived by two brothers and one sister; and several nieces and nephews.
