He was predeceased by: his parents, Lewis R. Stevens and Delores J. Stevens; his daughter Kari Jo Stevens; his brother John L. Stevens; his niece Pamela J. Myers; and his nephew Brian R. Herman.
He is survived by: his siblings, Carolyn M. Lee, Ronnie Underwood, Peggy Day (Wayne) and Sandy Langley (Terry); and his nieces and nephews, Cindy Jones, Jennifer Underwood, Valerie Darwin, Brandi Boggs, Krystal McKay, Mike Jones, Samuel Cavallaro and Doug Driskell. He is also survived by several great nieces and nephews and other extended family.
