He is survived by: his wife Marla Hazen; his stepson Justin Cook (Liea); his granddaughter Maci; his mother-in-law Ann Richards of Lampe; and his siblings, James "Jim" Hazen of Grinnell, KS, Richard "Dick" Hazen (Dee) of Salina, KS, Ron Hazen (Deb) of Cheyenne, WY, Ginger Seibel of Salina, KS, Marie Schmidt (Virgil) of Golden and Karen Boys of Logan, KS.. He is also survived by many nieces, nephews, and great nieces and nephews.
