He was predeceased by: his father James Perkins. He is survived by: his mother Shirley Perkins; his son Zachary; his wife Tracy; his daughter-in-law Alexandria Perkins; his son Tyler Perkins; and his brother Jason Perkins. He is also survived by several cousins, nieces and nephews, and brothers and sisters-in-law.
