He was predeceased by: his father Jim Ray Maize; his brother Jim Ray Maize; his stepfather Charles Epperson; his stepbrothers, Michael and Charlie Epperson; his grandparents, Harley, Helen Maize, Russell and Sara Thudium; his father-in-law Darrell Lloyd; his sister-in-law Sherry Lloyd; his aunt Shurlene Tooley; his uncle Cecil Thudium; and his cousins, Charlotte VanSickel and Terry Tooley.
He is survived by: his mother Helen Epperson of Ottumwa, Iowa; his wife Debbie Lloyd; his son Chad Jay Maize of Kirksville; his mother-in-law Hallene Lloyd of Kirksville; his brothers, Steve Lloyd, Lavern (Vickie Blackorby) and David (Tracie Lloyd); his aunt Sherry; his uncle Rick Munden; and his cousins, Tara Miller, Jason Munden, Eugene Thudium, Jamie Thudium and Belinda Tooley Lambright. He is also survived by numerous nieces, nephews, great nieces and great nephews.
