He was predeceased by: his parents, Jack D. McIntosh and Evelyn McIntosh (Ailes); and his siblings, Gail Curtiss, Marvin and Jim.

He is survived by: his wife Debbie Elaine Wheeler. He is also survived by several nieces and nephews.
