He is survived by: his wife Sherry Mustapha (Wells); his niece Dannette Libbee of Springfield; his brother-in-law Ross Wells (Pat) of Wildwood; his niece Michelle Farmer (Tom) of Wildwood; his nephew Scott Wells of Ann Arbor; his great-niece Karen Farmer; his great-nephew Mark Farmer (Lauren); and also, Pam, Katie, Dezo and Renee.

In lieu of flowers, memorial contributions may be made to First Baptist Church of Kansas City, Missouri in Jerry's name.
