He is survived by: his wife Geneva; his sons, David Cusack of Joplin and Philip Cusack (Karen) of West Hills; his grandchild Brian Reed (Sara); his great grandchildren, Keaton Penna of Little Elm and Charlie of Little Elm; his brothers, Michael Cusack (Mary Jane) of Joplin and Larry Cusack (Larry Downard) of Portland; and also, Margaret Reed (Parker) of McKinney, Elizabeth Cusack of Bella Vista, Jennifer Cusack (Carrie) of Camarillo, Jessica Penna (Joey), Parker Reed of Los Angeles, Jared Cusack, Camille Cusack of West Hills and Neil DeGondea of Bella Vista.

Of Joplin, Missouri passed away Friday, March 2, 2018 at his home following a long battle with Alzheimer's disease.
