With heavy hearts, we announce the death of Jay Polen (Joplin, Missouri), who passed away on July 26, 2019 at the age of 23. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
He is survived by: his parents, Richard Polen and Brenda Polen; his sister Rachael Polen; and his grandparents, Thomas and Geraldine Painter. He is also survived by extended family and friends.
In lieu of flowers the family requests donations be made to Autism Speaks in care of the mortuary.
