He was predeceased by: his parents, Clifford Eugene and Jane Parker Perkins (Forgey); and his daughter Jennifer Lane. He is survived by: his wife Elizabeth Catherine; his sons, Thomas Jeremy Perkins (Anne Marie) and Jonathon Michael Perkins (Malorie); his grandchildren, Charlotte, Thomas Jr., August, William and Brooks Perkins; his brothers, Mark, Craig and Tracy Perkins; and his half-sisters, Connie Wade and Janan Huntsberry (Bob). He is also survived by numerous nieces and nephews.
