He was predeceased by: his parents, Forest Clark and Georgia Clark (Phillips); and his wife Patricia Clark. He is survived by: his stepchildren, Kelly Surface, Tammy Hazelton and Aaron Tyson; and his stepgranddaughter Shehan Nichole Hazelton. He is also survived by two other step-grandchildren; three step-great-grandchildren; several cousins; and numerous beloved friends.
