He was predeceased by: his parents, L. Wayne Mayfield and Ruth Ann Mayfield (Green); his brothers, Danny Mayfield and Gary Mayfield; his father-in-law John Muench; and his brother-in-law David Cook (Wayne Jones).
He is survived by: his wife Phyllis; his children, Chris Muench (Becky) of Jackson, MO, Dustin Mayfield (Dani) of Welcome, Maryland and Amanda Pensel (Ryan) of Gordonville, MO; his grandchildren, Dylan, Taylor, Case, Conrad, Addysen, Aubrey and Rhett; and his son Donny Ford. He is also survived by numerous in-laws, nieces and nephews.
