She was predeceased by: her parents, Elbert Chapman and Lena Rebecca Chapman (Martin); her children, Patricia Elaine Forinash and Dennis Ray Forinash; her siblings, Irene Barbee, Lucille Moss-St, Zelma Eaton, Gladine Bick, Hilda Chapman, Harry Chapman, Robert Chapman, Paul Chapman, Hadley Chapman and Henry Chapman; and her great grandchildren, Eloise Grace Forinash and Owen Thomas Forinash.
She is survived by: her sons, Dwayne Forinash (Linda) and Darrell Jacob Forinash (Nancy); her daughter-in-law Carla Forinash (Porter) of Colorado Springs, Colorado; her nephew Charles Chapman; and her husband James Forinash. She is also survived by eight grandchildren, ten great-grandchildren.
