He was predeceased by: his father Tom. He is survived by: his children, Brenden and Cevin; his stepson Joshua; his mother Karen; his stepfather Jerry; his brother Shawn; his stepbrother Richard; his sisters, Tiffany, Belinda and Diana; and his stepsister Vicky.
In lieu of flowers the family suggests contributions to Alcoholics Anonymous and the Midwest Organ Transplant Network.
