He is survived by: his parents, David "Cross Guy" Middleton and Sandra Middleton; his brothers, David Middleton (Amanda) of Fishers, IN, Joshua Middleton (Rebekah Eaker) of Imperial, MO and Timothy Middleton of Hillsboro, MO; his nieces and nephews, Gavin Middleton, Samuel Middleton, Joseph Middleton, Alex Ellsworth, Emilee Long, Elisabeth Eaker and Ashlea Smith; his cousins, Shane Thomas, Daniel McConnell, Stephanie Hawkins and Johnny Dean; and his close friend Parker. He is also survived by Several other nieces, nephews, cousins, and friends.
