He was predeceased by: his parents, Joseph Kozlowski and Dorothy Kozlowski; and his sisters, Nina Belcher (late Thomas) and Carol Sleczka (late Jerry). He is survived by: his wife Maureen Kozlowski; his children, Keith (Emma), Allison and Jason Kozlowski (Shelby); his grandchildren, Madden and Baylor Kozlowski; his siblings, Mary Knight (Edward), Jim Kozlowski and John Kozlowski (Linda); and his siblings-in-law, Thomas Murphy (Nancy), Patrice Grace (Wayne), Cheryl Brandt (David), Timothy Murphy, James Murphy (Joanne), Kathleen Eisenbeis (Kenneth) and John Murphy (Christy).
