Mayfield of Louisiana, MO; his grandchildren, Dalton, Krystlyn, Trey, Gabe, Alex, Mason, Gracelyn, Kelli, Lucas, Bailie, Cameron, Lydia and Ziven; and his siblings, Lois Bird, Linda Wintjen, Gary Culp, Sue Culp, James Culp and Lori Sullivan. He is also survived by 2 great grandchildren.
