She was predeceased by: her parents, James H. Forbis and Betty Jo Forbis (Poore); and her sister-in-law Debbie Forbis. She is survived by: her husband Tom Klocek; her children, Tom, Jr. Klocek (Kim), Tamara Prochazka (Brent), Joseph Klocek (Leah) and Jamie Klocek; her grandchildren, Brianna (Alex), Cody, Kelsey (Tyler), Ashley, Jenna and Nathan; her great grandchildren, Austin and Emmitt; and her siblings, Joy Juhl (Jim) and Terry Forbis. She is also survived by many nieces, nephews.
In lieu of flowers, memorial donations may be sent to the American Cancer Society or local pet shelter.
