It is with deep sorrow that we announce the death of Opal Ruth Forman of Fulton, Missouri, born in Centralia, Missouri, who passed away on May 14, 2022, at the age of 95, leaving to mourn family and friends. She was predeceased by: her parents, Larkin Calverin and Linnie Ellen Poff (Cleeton); her husband Weldon Warren Forman; her foster daughter Shirley Mayes Evans; her grandson Randall Forman; her great grandchild Cason Hampton; and her siblings, Calverin Poff, Essilee Hinten and Fern Brown.
She is survived by: her sons, Ron Forman of Fulton, Glenn Forman (Freeda) of Farber, Norman Forman (Marty) of Fulton and Todd Forman (Kimberly) of Springfield; and her brother Lyndall Poff (Janice) of Columbia. She is also survived by 14 grandchildren, two step-grandchildren, 24 great-grandchildren, and several great-great-grandchildren.
Memorial donations may be made to Friendship Baptist Church or New Hope Cemetery in rural Centralia.
