He was predeceased by: his parents, Chalmer "Hank" Forsythe and Delorse "Dorrie" Ann Forsythe (Duncan); and his grandparents, Bill, Ann Duncan, Melvin and Ina Forsythe.
He is survived by: his wife Sarah Forsythe (George); his children, Conner Forsythe (Samantha Franklin) and Hannah Forsythe (Jacob Glazer); his brothers, Danny Forsythe (Michelle), Randy Forsythe (Vicki) and Brett Forsythe (Patty); his mother-in-law Mary Ann George; his siblings-in-law, Beth McEntire and Brian George (Judith); and his pets, Duncan and Finley. He is also survived by many nieces, nephews, aunts, uncles, cousins.
